Family Risk Assessment

This  survey will assess your personal and family history information to determine if  you’re at an increased risk of cancer. You will receive your result immediately on the screen. If your result shows that you may be at an increased risk of cancer, then a meeting with a genetic counselor will be recommended.

Are you of Ashkenazi Jewish ancestry with a personal or family history of breast, ovarian, prostate, pancreatic and/or colorectal cancer?
Do you have a close relative who has had positive genetic testing and/or a diagnosis of a hereditary cancer syndrome?
Have you previously met with a genetic counselor or had genetic testing?
